For referring professionals & guardians

A clear path.
A considered decision.

A referral is the beginning of a conversation about a young person’s needs, the program’s fit and what must be in place before admission.

An illustrated conversation, with faces turned toward one another.
Clarity at every next step
  1. Initial inquiry

Call or email. Referrals typically come from social workers, care managers, clinicians or guardians.

  2. Eligibility and fit review

We review age, diagnosis and current clinical picture, care and supervision needs, risk information, custody or legal authority, school needs, the payer/authorization path, and bed/staffing fit.

  3. Initial clinical documents

Send the current Comprehensive Clinical Assessment, recent CCA addendum recommending Level III, Person-Centered Plan and a psychological evaluation if available.

  4. Full document collection

Use the admission portal to gather and send the required documents. Multiple uploads are welcome; you do not need to send everything at once.

  5. Intake and placement assessment

Assessment, a tour where appropriate, and decisions by the responsible parties help determine the next step.

  6. Authorization and readiness

Written payer authorization, a signed Service Order and admission paperwork are completed before placement is final.

An important distinction

A referral is not
an authorization.

Placement cannot occur without an MCO authorization.

The authorization path requires a recent Level III recommendation and a PCP with the required signed Service Order. See the complete instructions and document checklist in the portal.

Initial review typically takes 2–4 weeks after a complete referral is received. Tours can be arranged for guardians and referring agencies; a waiting list is maintained when the facility is full.
